Do communities have more of a responsibility to community health or global health?

It seems we have huge efforts helping people abroad (globally) when there are local people who need our resources just as much. What are your opinions? Why should global health be more important than local community health? All opinions are welcome.

1. Why does it need to be a choice of one or the other? However, even if you focused just on local problems, there are a host of them...heart, cancer, birth defects, diabetes, and all the diseases. You'd still have to focus on one of many.
